Effective May 1, 2009, Fox Valley Orthopedics must comply with the Fair and Accurate Credit Transactions Act of 2003 (FACTA) as amended in October 2007 to include Red Flag and Address Discrepancy Requirements.
This Act is designed to protect your personal health and financial data.
Therefore, as of May 1, 2009, we are asking all of our patients to provide a government-issued
photo ID or other proof of ID at registration.
