Care Team Coordinator

Location: position location(s) may vary based on need (Geneva, Elgin, Algonquin & Barrington, IL)

Status & Hours: Full-time (36-40 hours per week) & Part-time (20-29 hours, including evenings and rotating weekends).

Summary: Provides secretarial support for a surgeon clinic. Answers phones, registers/updates patient accounts and schedules appointments.

Essential Duties and Responsibilities

Checks in and checks out patients, ensuring completion of documentation.
Coordinates follow up care including MRI, Surgery, Pt, labs, etc.
Enters and maintains physician and PA schedules, including loading each month template and coordinating provider schedule changes as they arise
Maintains test result log, ensuring all patients receive results in a timely manner.
Answers phones and directs calls.
Registers patients or updates existing account information.
Schedules appointments for patients.
Coordinates patient correspondence; maintains up-to-date, accurate patient electronic charts.
Communicates with other departments regarding physician schedule changes and issues pertaining to patient care.

Qualifications:

The requirements listed below are representative of the knowledge, skill and/or ability required.

Education & Experience

Required:

High school diploma or equivalent.
Two (2) plus years of secretarial experience, preferably in a medical office setting.

Preferred:

Degree in secretarial/business related field.
