Attention: Worker's Compensation Patients

Prior to your appointment, you must provide the name and telephone number of the appropriate contact at your employer and a record of your health insurance coverage. We require this information in order to confirm proper billing arrangements.

Work-related injuries need to be pre-approved by the employer or insurance company prior to your arrival and prior to 4:30 p.m. in order to ensure that provided care is eligible for coverage. Patients with work-related injuries who come to OrthoFirst™ without pre-approval or after 4:30 p.m. or on Saturday may be asked to self-pay for services rendered. A refund for self-pay services will be issued upon payment to Fox Valley Orthopedics from the worker’s compensation or commercial carrier.

Financial Arrangements

All co-pays, deductibles, and co-insurance are due at the time of your visit or procedure. We accept:

Self-Pay Patients

If you are paying for services yourself (self-pay), we require a base payment of $200.00 for an evaluation. If you require treatment that exceeds your base payment, a representative from our Patient Accounts Department will call you to discuss additional payment arrangements.
